In 1993 the Florida Legislature enacted the “Florida Americans with Disability Accessibility Implementation Act.” 
 
Affidavit of Gas Piping Test 
Submit an "Affidavit of Gas Piping Test" to certify that the piping has been tested for pressure and checked for leaks in the system. This affidavit will be required at the final inspection.

New Construction, Renovations, and Additions
 
 
Sign Permit Permanent 
The following must be included: Drawings to scale, showing proposed sign with dimensions and must show placement on elevations for wall signs; Site plan of parcel showing placement of signs, setbacks from right-of-way, and setbacks from side property lines. Must show setbacks between signs on same parcel; and a Letter of Authorization. 
 
Sign Permit Temporary 
Please include an attached drawing of signage including proposed copy. 
 
Swimming Pool - Spa Requirements 
Swimming pool, spa, or hot tub will be constructed and will meet the requirements of Chapter 515, of the Florida Statues.

Submit an "Affidavit of Compliance" with an attached "Notice of Commencement" to the clerk of the circuit court for recording.

Contractor Information

Contractor Certification Application 
Please submit a $75 Non-Refundable Application Fee, Renewable Annually. 
 
Contractor Certification Requirements 
If someone other than the qualifier is going to pull the permit, we need a notarized Letter of Authorization stating that individuals position with the company, giving them permission to pull the permit. 
 
Four Year Certificate of Competency 
Four years experience shall be experience gained through working as an apprentice or helper in the appropriate trade for four years or three years of education in a recognized accredited college or vocational school, plus one year of actual experience in the applicable trade. 
 
Notice of Election to be Exempt 
By filing this application, you elect to be exempt from the provisions of Chapter 440, Florida Statutes and waive any right you may have to workers' compensation benefits in the State of Florida should you become injured on the job. 
 
Notice of Election to be Exempt Instructions 
The following classes of construction industry business owners are eligible for exemption from the provisions of Chapter 440, Florida Statutes (Florida's Workers' Compensation Law). 
 
Two Year Certificate of Competency 
Applicant shall submit proof of having worked in their appropriate trade as an apprentice or helper for a minimum of two years.

Certificate of Appropriateness Application 
If applicant fails to notify staff and does not attend the meeting, the C.A. Application may be tabled for one month. If applicant fails to notify staff and does not attend the next monthly meeting, the C.A. Application may be denied without prejudice. 
 
Tree Removal Application 
A permit is required for any tree(s) 4 inches in diameter or greater, regardless of species or condition: EXCEPT for a parcel that is zoned R-1 and is less than 3 acres and is occupied by an existing residence. Phone 629-8404 to check the zoning. Parcel ID number & site address are required. 
 
Tree Restoration Plan Application 
Required replacement trees must be Florida Grade #1 or better based on Grades and Standards for Nursery Plants, Florida Department of Agriculture and Consumer Services, 2nd Edition, 1998.
