Service Disconnect Request Form
Information needed on this form can be found on your utility bill. Disconnects normally will occur between 8 a.m. and 5 p.m. on the day requested. A specific time cannot be guaranteed. If you have previously entered a disconnect and need to make a change or cancel your disconnect, do not re-enter the information. Please call our Customer Service Center at 629-8411 or 1-800-893-4760 during normal business hours.
If you wish to transfer your service from one address to another, do not enter a disconnect request. Please call our office for the transfer request.
*(Customers should provide one phone number for contact purposes. This number is how OEU identifies the actual address for your electric utility service in our outage management system. For commercial customers it should be your business number. For residential customers it can be your home or cell number.)
